Chronic Disease Management
Wirraka Maya provides coordinated, culturally safe care for people living with long‑term and chronic health conditions. Our approach focuses on prevention, early identification and ongoing management, supporting people to stay well and maintain quality of life over time.
Chronic disease management is delivered through a team‑based model, with Aboriginal Health Workers and Practitioners, nurses, doctors and allied health professionals working together to support regular monitoring, follow‑up and continuity of care. Services are designed to respond to individual needs while recognising the importance of family, culture and community in supporting long‑term health.
Living with a chronic condition can involve regular appointments, tests, medications and lifestyle changes. Wirraka Maya works alongside clients to help them understand their condition, feel confident in managing their health and access the right supports at the right time.
Support may include care planning, reminders for health checks, assistance attending appointments and referrals to other services such as allied health or visiting specialists. Wherever possible, care is delivered in a way that fits around people’s daily lives, family responsibilities and cultural obligations.
Monitoring, screening and prevention
Regular screening and monitoring are a key part of chronic disease management at Wirraka Maya. Our team supports early detection and ongoing assessment for conditions such as diabetes, heart disease, kidney disease, high blood pressure and other long‑term health concerns.
By identifying changes early, we can help reduce the risk of complications and connect people with additional support when needed. Where specialist input is required, the clinic team works closely with visiting providers and external services to coordinate care and reduce barriers to access.
